Grievance Submission Form

Alliance for Strong Families and Communities members may use the form below to file a written grievance with the Alliance if they feel their rights have been violated, or if they wish to appeal a decision they are dissatisfied with regarding a service they are being provided.

Members will be notified in writing of the disposition of their grievance by the Alliance’s COO within seven working days of the receipt of the written grievance.
